CarePartners of Georgia was established in 2003 and was founded and based upon the fundamental belief that individuals can and do recover from mental illness.

Our Story

David Crooke Kathy Durden

With over 25 years of experience in the behavioral health field, David and Kathy founded a company based on childhood lessons with a model of care that supports people’s ability to learn and grow, to be treated with dignity and respect, build’s on strengths of individuals.  The Recovery Model and the principles of recovery are the core foundation of every decision made within the company. The name “CarePartners of Georgia” symbolizes the unique experience of partnering with the individual, their family members, and any community partners such as: the Local School System, Department of Juvenile Justice, The Department of Family and Children Services, Providers, other identified community members, supporters, friend, any one the individual feels will support their recovery, their hopes, and their dreams. CarePartners of Georgia is committed to person-centered collaborative care. Our focus is not “what is the matter with the individual” but “what matter’s to the individual." Our team of staff understands their mission is to be a partner in someone else’s journey.

At CarePartners of Georgia, we provide quality, resiliency and recovery-based treatment while offering hope and healing to those with behavioral health issues. Outpatient, treatment for adults, young adults and children are available. Our culturally diverse team is carefully selected and composed of licensed counselors and social workers, bachelors-level case managers, community support workers, all whom are directed by clinical supervisors. We carefully coordinate a individual recovery and resiliency plan tailored to each individual’s needs. We gladly accept referrals!

As longstanding members of Bulloch, Candler, Emanuel and Johnson county communities, we work closely with our network of valued stakeholders. This close partnership ensures that our individuals make a strong transition into their communities with strategies that work in their world. CarePartners of Georgia experienced team stands ready for the next opportunity to make a difference. We work with various social agencies including the Georgia Department of Family and Children’s Services, the Georgia Department of Juvenile, the Georgia Department of Corrections, local county Boars of Education, State Courts Primary Care Physicians and many others.
